Off-Campus Forms:
Parental Permission Form (must be notarized)

Returning Student:
Please read the Heritage Academy Bulletin (Handbook) and Heritage Academy Standards before filling out the Parental Commitment; form is to be completed by the parent.

The following forms need to be completed and turned in before your arrival. You may choose to mail, fax or email the completed forms to the registrar.
- Sign the Parental Permission Form (must be notarized)
- Complete the Consent for Medical Treatment Form (must be notarized)
- A physician is to fill out and sign the Immunization Record
- A physician is to complete the Medical Examination Form
- Complete the Medical History Form
- Complete the Medication Administration Form
- Sign the Disaster Response Permission Form
- Sign the Cell-Phone Use Agreement
- Sign the Internet & Computer Agreement
- Sign the Dorm Alarm System Agreement
- Read and Sign Enrollment Contract
- A copy of the student’s social security card
- A copy of the student’s birth certificate
- Take or mail the Transcript Request Form to the registrar of the applicant’s last school.
